Introduction

See below for forms relating to consent for Use of Photography and Video, Monitoring and Support Groups, Risk Assessments, Safeguarding Contracts and Safeguarding Concerns.   There is also an acceptable use policy for guest wifi.   If you have any questions about the forms, these should be directed to safeguarding.

Acceptable Use Policy for Guest Wi-Fi

The acceptable use policy should be displayed, or be accessible to users by means of a prominent hyperlink, before the user gains access to the Wi-Fi network.

The Acceptable Use Policy/the terms and conditions of the contract must be available in a way that allows the user to store and reproduce them.  Encouraging users to print terms and conditions is a common way of doing this.

Consent Forms for Use of Photography and Video:

There are 2 consent forms, one for parents and carers, and one for young people 13 and over.   

These forms have been updated to ensure that our consent obligations are increased and to include reference to our privacy policy as well as to reflect a change in legal age of young person’s consent. 

For young people 13 and over, both the young person and the parent or carer will be asked for consent, while for those 12 and under, a parent or carer will need to consent on their behalf.
